Water Survey

Dear Gassville Water Customer,

Our water system, along with all other public water systems across the United States are conducting a survey of water service line materials classification as required by the Environmental Protection Agency under "Service Line Inventory Requirements of January 15th, 2021 Lead and Copper Rule Revisions." Enclosed you will find a survey for you to complete to help us out with some required information on your side of the water system. Please take the time to fill it out and mail it back in the prepaid postage envelope included. You can also drop it by City Hall, or in the overnight drop box at the drive through. If you are unsure of the answer to any part of the survey, that is ok. It is ok to put "unknown". If unknown, a Licensed Water Operator will be required to come by and verify the type of service line you have on your side of the meter. It is possible they may have to do some minor excavation right at the meter box to determine the type of line. We are required by law to submit the results of this survey to the Arkansas Department of Health by October of this year. We appreciate your time and cooperation with this brief survey.
